- Integrated care processes — VillageHealth provides a range of ancillary services required by CKD and ESRD patients including specialized pharmacy and lab, vascular access centers, targeted patient education and preventive care interventions. Easier access for patients means easier coordination for providers.
- Patient-centric nurse coaching — Each patient is assigned a dedicated nurse coach, also known as a VillageHealth Nurse or VHN, who completes a comprehensive needs assessment that includes a rigorous measure of the patient’s activation level. The VHN then develops an individualized care plan based on evidence-based kidney-care interventions, and supports the patient’s progress toward self-care with proven motivational interviewing approaches.
