Accountable Care Organizations (ACOs) is a hot health care issue, but VillageHealth’s NCQA-accredited integrated care management program has been operating and continuously improving since 1997. We work with private and government health plans to provide care management services to more than 5,900 individuals with chronic kidney disease (CKD) and end stage renal disease (ESRD). We work with nine commercial health plans and operate two demonstration programs for the Centers for Medicare & Medicaid Services (CMS).
At the heart of our program is the VillageHealth Nurse (VHN), an experienced nephrology nurse who develops, monitors and implements an individualized care plan for each patient. The VHN is in charge of coordinating the patient’s various providers, and helps the patient overcome such barriers to care as making appointments and arranging transportation.
VillageHealth improves outcomes across all stages of CKD by focusing on such drivers of unnecessary hospitalizations as infection, medication errors and fluid overload. Our outcomes include dramatic reductions in catheter use, enhanced medication compliance and lower hospitalization rates.
