VillageHealth Disease Management offers programs for renal patients who have CKD but are not on dialysis as well as patients who have developed ESRD requiring dialysis.
From VillageHealth nurses and nephrologists to financial analysts and technology specialists, we have assembled a cohesive core team of highly trained and qualified professionals whose diversified disciplines are unified to create success for our partners and their patients. The core team was designed to ensure the greatest success in patient outcomes and managing costs.
The four key elements:
- Advanced Care Management: The foundation of the VillageHealth Disease Management solution. It provides seamless coordination for patients' care. Unlike other disease management organizations, VillageHealth Disease Management offers field-based care in addition to telephone-based care. The nurses on our Core Care Team actually visit the patients in person-in their homes, at their physicians' offices, or at their dialysis units.
- Renal Delivery System Optimization: VillageHealth Disease Management offers unmatched clinical and quality analysis expertise. Our ability to make changes at the physician level is a key ingredient to our success. Our Regional Medical Directors, utilizing our extensive analytic resources and reporting capabilities, work closely with our Medical Advisory Board-composed of prominent nephrologists-to achieve the highest clinical standards. We will assist you in developing, reviewing, and disseminating guidelines targeted to achieve optimum efficiency, allowing for better overall patient care and optimal cost control.
- Medical Cost Analysis and Reporting: Critical to your success is our ability to extract data to identify opportunities for clinical improvement and cost savings. Our medical consulting team can assist with profiling your physicians to achieve excellence in your provider network.
- Technology: VillageHealth Disease Management developed a state-of-the-art information system, including a clinical care management tool called Medical Information System Technology (MIST) to remotely enter primary source clinical data into our central database.
Whether a patient is in the early stages of chronic kidney disease or at end-stage renal disease, VillageHealth Disease Management offers a care plan to educate patients and their caregivers as well as track outcomes and co-morbid conditions.
Meet the VillageHealth Core Team
Our core team of experts includes:
- Regional Medical Director (RMD), a board-certified nephrologist
- RMDs are experts in patient risk assessment, care management, and quality improvement, as well as development of provider and patient education
- VillageHealth Nurse (VHN), a locally based registered nurse with extensive training in renal care
- The VHN identifies the needs of the patient including medical, social, behavioral, emotional, and financial concerns through medical record review, in-person consultation with patient, family, and healthcare team members and acts as the link between the patient, physician, provider, and dialysis care team. The VHN ensures that healthcare activities are appropriate to the long-term needs of the patient, and are incorporated into a disease management plan that is mutually agreed upon by the member, physicians, dialysis care team, and any other providers involved.
- Regional Operations Manager
- The Regional Operation Manager interfaces with health plan representatives to ensure the smooth integration and ongoing relationship management of the VillageHealth Disease Management program.
