VillageHealth has grown into the nation's leading provider of kidney disease management services. We offer managed care organizations opportunities to both lower costs and improve clinical outcomes.
With trained nurses coordinating patient care, we can provide the education and tracking that chronic kidney disease (CKD) and end-stage renal disease (ESRD) patients require. Most CKD and ESRD patients have three to four comorbid conditions by the time their renal disease is detected, and these conditions are difficult to manage. We emphasize the leadership role of the nephrologists with the use of clinical guidelines and manage the continuum of illness from CKD through ESRD to allow for a seamless program of patient management. This leads to improved quality of life for patients, improved clinical outcomes, and lower overall costs.
The key services provided by this program include:
- A locally based VillageHealth Nurse (VHN), trained in renal care, is assigned to the patient and provides one-on-one service and support
- Comprehensive implementation process
- Evidence-based guidelines/management plan
- Identification and management of risk factors and comorbid conditions
- Provider community involvement in continuous quality improvement (CQI)
- Coordination of care among case managers, VillageHealth nurses, PCPs, nephrologists, and other specialists
- Field-based VillageHealth nurses supplemented by telephonic nurse capability
- Quality-of-life improvements for patient
- Process and outcome measures that have been benchmarked for quality improvement activities
- Patient health education to promote self-management of their conditions
- Data management tools
- Set financial and clinical targets associated with a risk structure
- A variety of standard analyses and reports available to monitor the program
