How Will VillageHealth Help Me?
As a VillageHealth member, you will be assigned a nurse (called a VillageHealth Nurse or VHN) who lives in your community and works directly with you. Your VHN is specially trained to care for people with kidney disease, diabetes, heart disease, high blood pressure, and those who need dialysis. Your VHN will work with you, your doctors, your pharmacist, and the healthcare team to develop and carry out a plan of care that meets your special needs.
How VillageHealth Disease Management Helps Improve Your Care
- We provide support and education for patients and their families
- We help coordinate care with your physicians and other healthcare professionals who may be treating other health conditions
- We help patients manage their disease in the early stages to help slow the progression of the disease
- We provide preventive care and healthcare screenings
- We help manage other health concerns to avoid hospitalization when possible
- We coordinate dialysis treatment with the patient's other providers
- We can review medications and provide guidance on appropriate therapy
What Exactly Does VillageHealth Do?
Our VillageHealth Nurses are specially trained to care for people with kidney disease, diabetes, heart disease, high blood pressure, and other health conditions. Depending upon your needs, they will:
- Help you manage your nutritional needs
- Work with you and your doctors to manage the medications that you take
- Monitor your health progress based on lab test results
Your VHN is available 24 hours a day to also:
- Answer questions when you are not feeling well and need advice
- Answer questions about your medicines, foods you eat, and treatment options
- Teach you and your family about diabetes, high blood pressure, the kidney, and the medical care given to manage your condition
- Help arrange for your vaccinations and routine check-ups
- Help you understand your doctor's plan for your care
- Visit you if you go to the hospital
- Visit you and your family in your home or at a healthcare clinic
- Direct you to someone in your health plan who can answer benefit questions
These services help improve your treatment experience and your overall health and well-being.
