VillageHealth Disease Management Provides Answers to Your Healthcare Needs
Q. How do I become eligible to participate in the program?
A. If you're a member of a participating insurance company, you may be eligible to benefit from the program. In this case, your health plan or your physician may refer you to VillageHealth DM as someone who could benefit from the program based upon their assessment of your medical condition You can decide not to participate in the program at any time.
Q. Do I have to pay anything to participate in the program?
A. No. The program is a free service provided by a participating insurance company.
Q. Will I still be able to see the same doctors?
A. Yes. Most health plans will allow you to continue seeing the same doctors.
Q. Would my doctor know I'm participating in the program?
A. Yes. Your health plan and VHN will contact your doctors to tell them you are in the program.
Q. How do I use the program's services?
A. To use any of our services, simply contact your VHN. Your VHN is your personal care manager and will answer any questions you might have. Your VHN is available 24 hours a day, seven days a week.
Q. What kinds of questions can I ask my VHN?
A. Call your VHN when you have any questions about:
- Your health
- Symptoms you may experience
- Medications
- Transportation
- Medical appointments
- Hospital visits
- Family or lifestyle changes
- Planned tests or surgeries
Q. Whom do I contact if I have a question or concern about my healthcare?
A. You can contact your VHN with any feedback, comments, or concerns regarding your care.
Q. What are the program's hours of operation?
A. Your VHN is available 24 hours a day, seven days a week.
Q. How does the program use my personal medical information?
A. Information about your health is kept confidential by VillageHealth DM. Your health plan, and your VillageHealth care team and your doctor are the only parties who can access your information. VillageHealth DM will not use your personal health information for any sales or third-party marketing activities.
Q. Does VillageHealth DM promote specific products as part of this service?
A. The program does not advertise, market, or promote specific products or services when discussing a patient's health condition.
Q. Will I continue to be on the transplant list?
A. Yes. The program and your health plan encourage you to complete evaluation and testing to remain active on the transplant list.
