What Is Disease Management?
VillageHealth Disease Management is a health coordination service offered by your insurance company at no extra charge to you.
Our people are available to make sure you get the care and attention you need. Our dedicated VillageHealth nurses work directly with you as they coordinate care with your doctors, your dialysis center, your pharmacist, and the other healthcare providers that you may use. Our team strives to make sure you to get the services that will benefit you the most and enhance your health and well-being.
At VillageHealth Disease Management, we offer an experienced team of specially trained nurses who are available 24 hours a day to meet your needs. Their expertise enables us to provide a capable and caring disease management program that is the largest in the nation. The program also helps you take an active role in managing your kidney disease while enjoying a long, satisfying, and full life.
