Partners for Better Patient Care
According to the Disease Management Association of America (DMAA), disease management is defined as a system of coordinated healthcare interventions and communications for populations with conditions in which patient self-care efforts are significant. VillageHealth Disease Management works with healthcare providers to coordinate patient care and provide solutions that improve patient outcomes and lower healthcare costs.
VillageHealth is retained by the patient's insurance company and works side-by-side with you to implement a comprehensive program for patients called Core Team Care. This program has elements that can assist both CKD and ESRD patients. Core Team Care partners with physicians, the dialysis center, and the insurance plan to provide every patient an individualized treatment plan carefully tailored to support his or her changing medical needs and therapies.
Whether you are a generalist or a specialist, your involvement in Core Team Care for your renal patients is critical to obtain the best outcomes possible. VillageHealth's core team is composed of a Regional Medical Director (RMD), a VillageHealth Nurse (VHN) and a Regional Operations Manager. VHNs are registered nurses with extensive training in renal care located in your community. Core Team Care is not a one-size-fits-all approach, but one formulated to meet the specific needs of each and every patient. We work with you and the entire care team to develop the plan for your patient, and once it is developed, we help to keep your patient on track.
