VillageHealth Disease Management’s coalition of experienced leadership enables us to achieve our mission improving the health and quality of life of renal patients; subsequently lessening the burden of healthcare costs.
- Our Corporate Management Team is a significantly accomplished group of people with deep experience in the healthcare and health insurance industry. They are focused on building a community of people sincerely committed to bettering the lives of patients suffering from kidney disease.
- Our Medical Advisory Board is comprised of a select group of some of the most recognized nephrologists in the nation. Under their guidance, VillageHealth adheres to strict clinical quality standards and continues to develop improvement initiatives that ensure our patients receive the best care possible.
- Our Regional Medical Directors are leading nephrologists who support providers in the individual markets. They also work with providers, health plans, and the VillageHealth Disease Management teams to identify trends in the local community, assess outcomes, and implement continuous quality improvement initiatives.
- Our VillageHealth Nurse (VHN) is at the heart of our medical model. Registered nurses with specialized renal training and extensive experience in disease management coordinate healthcare resources, facilitate communication and provide education to help kidney disease patients achieve the best health possible.
